Our office functions by appointments only. When calling for an appointment, you will be asked your name, to briefly describe the nature of your problem, and telephone number where you can be reached during business hours.

If you are a new patient and have had any testing, including x-rays and lab work, you will be asked to bring these reports with you at the time of your visit. All patients are asked to bring their medication to the office at the time of your appointment.

If you cannot honor your appointment, please extend the courtesy of contacting us immediately, but at least within 24 hours.

  DIAGNOSTIC TESTING
All diagnostic testing is performed on state of the art equipment and is by appointment only. If you are unable to keep your scheduled appointment, please extend the courtesy of notifying our office as soon as possible so that we may perhaps accommodate another patient. Certain tests require specific preparation prior to your arrival at the diagnostic center. If you are unsure as to your instructions, please ask the technician or nurse.   MEDICATION REFILLS
Please call anytime you need a prescription refilled. Please have your pharmacy name and telephone number ready. You will be connected to our voicemail for prescriptions and asked to leave a message. For mail away services, please call in advance. If you wish to drop off your prescription requests, you may do so and pick them up later. Prescriptions will be called into your pharmacy after 4:00 p.m.  Patients must be seen by one of our physicians at least yearly to have a prescription refilled.
 
  BLOOD PRESSURE CHECKS
Hours: Tuesday & Thursday 9:00 a.m. to 10:00 a.m. If your physician has ordered a blood pressure check to be done by the nurses and you cannot come between the times indicated above, you will need to make an appointment. This time is for blood pressure checks only. If you have other medical questions or concerns, please call the office for an appointment.
 
  COUMADIN PATIENTS
It is important that you call us if you have not received your Protime results within 24 hours after testing. Inform the receptionist of the date and place where the Coumadin level was drawn. REMINDER: It must be drawn monthly unless otherwise instructed.
 
  INSURANCE INFORMATION
Please present your insurance card at the time of each visit. NICP participates in numerous plans. As a service to our patients, we will file all insurance claims. You are responsible for your deductible and/or co-pay at the time of service. To keep our records up to date, we ask that you notify us of any address or insurance changes.
 
  DISABILITY FORMS
We request that all forms be brought to the office and left to be completed. Please allow enough time for your form to be completed prior to picking up. There is a $5.00 charge before the form is completed. You must fill in your portion of the form before bringing it to the office.
